Care plans are built for clients, so they are informed and therefore they consent to the treatment they will be receiving.A� It is imperative that the client be involved in the creation of the care plan, based on the human needs discussed in the previous blog, so as to have an understanding of what to expect and to have adherence to the appointments established for treatment.A� In addition to linking the dental hygiene diagnoses from the human needs assessment to the care plan, a dental hygienist also selects clinical and health promotion interventions that are indicated and beneficial to the client.A� It is these elements that the care plan is comprised of.A� The care plan is generated in conjunction with the client, supported by evidence-based practice (including the recare frequency),A�and is thereforeA�client specific.

When discussing the care plan with the client, it is important that privacy is ensured in the discussion environment; that all aspects of the treatment are clear to the client, as in the number, length, sequence and cost of each appointment, which are prioritized in conjunction with the client.A� Prior to the implementation of the care plan, it is necessary that the client is in agreement and consents to the treatment being proposed.A� This is verified again at the time the treatment is to be commenced, when informed consent is attained from the client.

When care plans are being implemented, it is required that previously established goals are evaluated and modified if necessary. Or, if the goals of an appointment are not met, modifications to the care plan would also be indicated.

It is in the care plan that any referrals to other dental health care providers are also noted.
